Two cutting edges or one? A practical comparison of universal and Gracey curettes, with the differences between the four classic universal designs.

A universal curette has two cutting edges and a blade set at 90° to the lower shank. It can be adapted to every tooth surface – mesial, distal, buccal and lingual – with the same instrument, which makes it the efficient choice when you want few instruments on the tray. But "universal" covers four quite different classic designs, and each has a job it does best. This guide compares them and explains when a universal curette beats a Gracey – and when it does not.

The two are complementary, not competing. A typical tray carries one universal curette for broad work and two to four Gracey numbers for deep pockets. The four classic universal designs

Columbia

A short, sturdy blade on a moderately angled shank. Columbia 13/14 is the posterior workhorse; Columbia 4L/4R and 2L/2R are designed for anterior teeth and premolars. The L/R marking means the two ends are mirror images, so one instrument covers both sides of the arch. Langer

Langer curettes combine a universal blade with Gracey-style shank angulations: Langer 1/2 for mandibular posterior teeth, 3/4 for maxillary posterior teeth and 5/6 for anterior teeth and premolars. They suit operators who like the reach of a Gracey shank but want both edges available. Available as Langer 1/2, 3/4 and 5/6, mini Langer 1/2, 3/4 and 5/6, and Langer 1/2 in titanium.

Barnhart

A longer blade on a slightly angled shank, used on all surfaces – particularly posterior teeth with moderate pockets. Available as Barnhart 1/2 and 5/6.

McCall

A small blade on a short shank, for posterior teeth and tight areas – narrow interdental spaces, furcations and narrow roots. Available as McCall 13S/14S, 17/18 and 17S/18S, plus mini McCall 13S/14S.

How to choose

  • Few instruments, moderate calculus: Columbia 13/14 for posterior teeth plus Columbia 4L/4R for the anterior region.
  • Deeper pockets with a universal blade: the Langer series – shank angulations like Gracey 5/6, 11/12 and 13/14, but with two cutting edges.
  • Tight interdental spaces and narrow roots: McCall 13S/14S or a mini Langer.

Technique notes (design, not protocol)

Because both edges of a universal curette cut, the blade is adapted with the face at roughly 90° to the tooth and the lower shank parallel to the surface being scaled. On a Gracey the lower shank is held parallel to the root surface and only the lower cutting edge engages. Which technique a clinician uses is a clinical decision; the point for purchasing is that a universal blade forgives small angulation errors better, while a Gracey rewards precision with better adaptation in deep pockets.

Sharpness

Common mistakes when buying universal curettes

  • Buying one design for everything. Columbia 13/14 is excellent posteriorly but its short blade is awkward on long anterior roots; pair it with 4L/4R.
  • Ignoring shank length. Langer shanks reach deeper pockets than Columbia; if your patients are mostly maintenance cases with shallow pockets, the shorter Columbia shank is easier to control.
  • Mixing handle sizes. When one universal is ErgoX Plus and the Graceys are ErgoX, the grip changes with every instrument. Standardise.
  • Using steel universals on implants. Columbia 4L/4R and Langer 1/2 exist in titanium for that purpose.

Setting up a tray for a new clinic or a new hygienist

A practical minimum for periodontal maintenance is one universal (Columbia 13/14), one anterior universal (Columbia 4L/4R), Gracey 5/6, 11/12 and 13/14, a sickle scaler (H6/H7 or 204S) and a probe/explorer combination such as explorer 23 + CP-12. That is seven instruments and covers routine scaling for most patients; deep-pocket cases add mini Gracey 11/12 and 13/14.   • Universal curette – two cutting edges, blade at 90° to the shank, usable on all surfaces.
  • Area-specific curette – the Gracey family: one cutting edge, blade offset about 70°, shank designed for specific surfaces.
  • Toe – the rounded end of a curette blade (a scaler has a pointed tip instead).
  • Terminal shank – the part of the shank nearest the blade; its angle to the tooth guides adaptation.
  • L/R, S – mirrored ends (left/right); S denotes a smaller blade in the McCall series.

What does 4L/4R mean on a Columbia?

Can I replace all my Gracey curettes with universals?
